Liability Agreement for Calendar Year 2023

I the parent/legal guardian of this child do hereby release Hope United, Inc., its agents, assigns, employees and volunteers from any liability whatsoever arising out of injury, sickness, damage or death which may be sustained by the above named minor during any activity, field trip, function or the like sponsored by or participated in by Hope United, Inc. I also give my permission for the above-named minor to attend and be transported to any activity, trip, function or the like sponsored by Hope United, Inc. You may reserve the right to review the release of liability form in it’s entirety by written request to admin@hopeunited.church before agreeing.
